Looking Through the Nervous System: A Neurological Approach for Ophthalmologists

For the veterinary ophthalmologist, the neurological examination is a vital extension of the eye exam. Many visual and ocular motor abnormalities arise from lesions affecting the brain and/or cranial nerves rather than the eye itself. A focused neuro-ophthalmic assessment helps determine whether vision loss, anisocoria, or abnormal eye movements are due to ocular disease or neurological dysfunction. This examination emphasizes evaluation of cranial nerves II–VIII, assessment of vision and menace response, pupillary light reflexes, ocular motility, and observation for proprioceptive deficits that aid in lesion localization. Mastering these neurological components enables ophthalmologists to recognize neurological causes of ocular signs, improving diagnostic accuracy and case management.
